It is a time for you to reflect on your values and wishes\, and to let people know what kind of health and personal care you would want in the future if you were unable to speak for yourself.\n \n\n	It means having discussions with family and friends\, especially your Substitute Decision Maker – the person who will speak for you if you cannot speak for yourself. It may also include writing down your wishes\, and talking with healthcare providers and financial or legal professionals.\n \n\n	This year’s National Advance Care Planning Day theme is #TeamSupport – who will support and care for you when you are ill? DESCRIPTION:Location: Bethell Hospice Living Room\n \n\n	Facilitators: Nazira\, Linda\, Carolyn\n \n\n	In preparation for a workshop presentation at the HPCO conference in April\, our team will present the results of a qualitative research study conducted at Bethell Hospice. DESCRIPTION:Second of two-part workshop that helps staff and volunteers communicate more effectively with individuals and families. The workshop consists of mini-lectures\, interactive exercises\, videotaped case studies and skill practice with peers to provide individuals with specific and efficient communication strategies that can be utilized to support client-centered care. The first part is scheduled for January 24.\n\n\n	\n	Objectives\, at the end of the workshop\, participants will be able to:\n\n\n\n		Reflect on the role of the staff or volunteer as a facilitator of change\n	\n\n		Identify strategies to assess an individual’s motivation & readiness for change using Motivational Interviewing techniques\n	\n\n		Learn communication skills and techniques to deal with individual and families\n	\n\n\n	Open to staff\, volunteers\, and the public.
