Social Work Supports

Each member of our professional Social Work team is registered with the Ontario College of Social Workers and Social Service Workers.

Social Work is a profession concerned with helping individuals, families, groups and communities to enhance their individual and collective well-being. Social Work support is comprehensive and focuses on whole person care. It considers an individual’s well-being in the areas of physical health, emotional health, mental health, social life, family life and financial stability.

Social Work supports and services include:

  • One-to-One Supportive Counselling
    • living with acute and chronic health challenges; coping with illness-related anxiety and depression; adjusting during times of transition; dealing with anxiety related to testing and procedures, and with fear of dying; facilitating legacy work projects
    • managing caregiver responsibilities and avoiding burnout; dealing with anticipatory grief
    • coping with bereavement, loss & grief
  • Support Groups
  • Assistance navigating the health care system
  • Advanced Care Planning
  • Patient Advocacy
  • Guidance on funding sources & assistance with the application process
  • Linkages & referrals to complementary supports
  • Education & Resources
